We Need Your Input!

Pharmacy Services Inc. needs your input! We are working to identify needs within community pharmacies, as well as health-system outpatient pharmacies and ambulatory care clinics, so we may provide solutions to meet those needs. We have two surveys to gauge your interest in different topics. The first survey covers collaborative practice agreements (CPAs) and the other medication therapy management (MTM). Please take a few moments to fill out one or both of the surveys. If you include your contact information on the survey (it is optional), we will send you a small token of our appreciation as a “thank you” for providing your input.
